Draft DPA Application Wins Award!

For the past year, we have been revising our Application for Services (Gen50B) form using the expertise of the Kleimann Communications Group.

Although still in draft form, Kleimann entered it into the MarCom Creative Awards competition in October… and WE WON! Our draft Application for Services won the MarCom Platinum Award in the Writing/Technical category. Only 15% of entries received this most prestigious Platinum Award this year.

The MarCom Creative Awards is an international competition for marketing and communication professionals involved in the concept, writing, and design of marketing and communication programs and print, visual, and audio materials. Past winners include individual communicators, media conglomerates, and Fortune 50 companies.

The project began with a two-day Forms Design Training Workshop that provided the Application Project Team the necessary tools to begin the task of redesigning our application form.

The Application Project Team membership includes:

  • Clarissa Moon (Field Services)
  • Kathy Ensor (Policy Section)
  • Eileen Monaghan (Systems Operations)
  • Jim Steele (Policy Section)
  • Karen Aaltonen (Policy Section)
  • Elizabeth Lebert (Policy Section)
  • Claudia Cook (Operations Support)
  • Carolyn Spalding (Policy Section)
  • Clay Butcher (DHSS Publications Office)
  • Venietia Santana (Policy Section)
  • Anne Bacinski (DHSS Publications Office)
  • Joan Chase (Policy Section)

A field user group consisting of staff throughout the state reviewed an early draft and provided the team with valuable suggestions for further improvements.

The Application Project Team is proud of this accomplishment and look forward to introducing our new application form to our customers and staff. The changes made to the form will make it easier for applicants to complete as well as for workers to collect the important information needed to make thorough eligibility determinations. Our plan is to begin using this new application this summer.
