The AD #2 form, Preliminary Examination for Interim Assistance, has been revised. This form is used as part of the process for establishing eligibility for Interim Assistance. It has been revised to update the SGA (Substantial Gainful Activity) amount from $780 per month to $800 per month as part of the annual cost of living adjustments for 2003.

Please begin using the new 1/03 version of the AD #2 form beginning January 1, 2003,and destroy or recycle any previous versions.
