103-2 IDENTIFIABLE APPLICATIONS
DPA cannot require applicants to use a specific form to request benefits. The most common way a household requests benefits is by submitting the first page of the Application for Services (GEN 50C) that contains their name, address, and signature.
Although applicants can submit a request for service to secure their benefit start date, a completed application is required before benefits can be issued.
When DPA receives an incomplete but identifiable application, it must be registered in EIS to protect the benefit start date.
Examples of when an incomplete but identifiable application will be registered in EIS are explained below.
An application comes in to a DPA office and cannot stay to complete the entire application. The applicant may complete and sign page 7 of the GEN 50C and submit it as an identifiable application for all programs requested.

If using page 7 as an identifiable application for all programs, staff will need to make a copy of page 7 and return the entire application form to the applicant.

A Senior Benefits or MAGI Medicaid application must be registered as an application and not as a request for service. Refer to MS 126-2 A for additional information.
103-2 A. REGISTERING AND FILING IDENTIFIABLE APPLICATIONS
When an applicant submits page 7 of the GEN 50C that DPA accepts as an identifiable application, the rest of the GEN 50C is given back to the applicant to complete. Staff must date stamp the identifiable application with the date of receipt, and register it on EIS . Date stamping the identifiable application protects the date the applicant initially expressed the need for having their eligibility considered.

If the identifiable application comes through electronic means, ILINX will date stamp the date received when submitted through the Capture tab.
When a recipient submits a GEN 72 after the month following the end of the certification period that DPA registers as an identifiable application, a GEN 50C must also be completed before DPA can authorize benefits for the recipient.
Example:
Certification ends in October. The GEN 72 received in December, which is after the month following the end of the certification period, is considered a late recertification and must be registered. A completed GEN 50C is required before benefits can be authorized. This example does not apply to MAGI Medicaid. Refer to MAGI Medicaid MS 806-3 for more information about late renewals for MAGI Medicaid.
When a recipient submits a GEN 72 and requests to add another program to their case, the request must be registered as an identifiable application. A GEN 50C must be completed before DPA can authorize benefits for the recipient. A completed GEN 50C is not required for MAGI Medicaid.
103-2 B. NOTICE AND INTERVIEW REQUIREMENT
DPA will notify applicants who submit an identifiable application of the interview requirement. Staff must send the N000 (NOTICE OF SCHEDULED INTERVIEW) if the cold call is unsuccessful. The N000 notice informs the applicant of the receipt of their application, a reminder to complete the entire application packet, and the requirement to attend an interview.
Note:
N000 is sent when an interview is required. The notice is sent on each case number associated with the program that requires an interview.
Example: If a client applies for SNAP , APA , APA -ME , ATAP , and GRA , staff must send the N000 notice on each case number, not each individual program. See Admin Manual MS 103-3 Case Number on EIS and ARIES for more information on how case numbers are assigned.

If an applicant fails to attend an interview and complete the application process, their application will be denied following program specific rules.

An interview is not required for Senior Benefits or MAGI Medicaid.
