701-8           WHEN A FAMILY WITH MORE THAN 57 MONTHS DOES NOT WANT AN EXTENSION

 

In this situation, the Work Services case manager attempts to work with the family on a plan for their exit from the program.  The exit plan describes referrals to resources or services available to the family after they leave assistance. A copy of the exit plan and applicable forms (TA15, TA16, and TA20) are put in the case management file, and a CANOCase Notes notation is made indicating why the family did not request an extension.  See Administrative Procedures MSManual Section 109-10 for additional CANOCase Notes requirement guidance.

 

The caseworker is informed of the extension results and acts to close the case with timely adverse notice.
