122-12 SUBMITTING DIRECT DEPOSIT ENROLLMENT FORMS
The Benefit Issuance & Recovery (BIRU) unit EBT Help Desk is responsible for processing direct deposit enrollment. Upon receipt, the DPA direct deposit enrollment form is to be uploaded and marked "worked/completed" in the document imaging system (ILINX) before submitting the email request to the EBT Help Desk at DPA_EBT@alaska.gov. Include PI's name, case number, client ID, and the date the document was received in the email. BIRU can access the form through the Content Store.
Note:
Original enrollment forms do not need to be forwarded to BIRU. It is not necessary to include any EIS screen prints with the Enrollment form. When possible, Direct Deposit Enrollment forms should be checked for completion before uploading to ILINX ‒ this applies to walk-in clients completing the form at the DPA offices.
