122-12 SUBMITTING DIRECT DEPOSIT ENROLLMENT FORMS
The Benefit Issuance & Recovery (BIRU) unit EBT Help Desk is responsible for processing direct deposit enrollment. Upon receipt, the DPA direct deposit enrollment form is to be uploaded and marked "worked/completed" in the document imaging system (ILINX) before submitting the email request to the EBT Help Desk at DPA_EBT@alaska.gov. Include PI 's name, case number, client ID, and the date the document was received in the email. BIRU can access the form through the Content Store.
Note:
Original enrollment forms do not need to be forwarded to BIRU . It is not necessary to include any EIS screen prints with the Enrollment form. When possible, Direct Deposit Enrollment forms should be checked for completion before uploading to ILINX ‒ this applies to walk-in clients completing the form at the DPA offices.
