122-12        SUBMITTING DIRECT DEPOSIT ENROLLMENT FORMS

 

The Benefit Issuance & Recovery (BIRU) unit EBTElectronic Benefit Transfer Help Desk is responsible for processing direct deposit enrollment. Upon receipt, the DPADivision of Public Assistance direct deposit enrollment form is to be uploaded and marked "worked/completed" in the document imaging system (ILINX) before submitting the email request to the EBTElectronic Benefit Transfer Help Desk at DPA_EBT@alaska.gov. Include PIPrimary Information Person's name, case number, client ID, and the date the document was received in the email. BIRUBenefit Issuance & Recovery Unit can access the form through the Content Store.

 

Note:

Original enrollment forms do not need to be forwarded to BIRUBenefit Issuance & Recovery Unit. It is not necessary to include any EISEligibility Information System screen prints with the Enrollment form. When possible, Direct Deposit Enrollment forms should be checked for completion before uploading to ILINX ‒ this applies to walk-in clients completing the form at the DPADivision of Public Assistance offices.
