1008-4          EVALUATING THE FSSPFamily Self Sufficiency Plan FOLLOWING THE EXTENSION REVIEW STAFFING

 

If the client is not engaged in work activities which are consistent with their self-sufficiency goals, case managers use the Extension Review Staffing discussion to work with the client to strategize approaches to successfully exit ATAPAlaska Temporary Assistance Program.  The FSSPFamily Self Sufficiency Plan (WSPM 1002) will be updated based on the Extension Review Staffing within ten (10) business days, unless the Extension Review Staffing indicates that all activities on the current FSSPFamily Self Sufficiency Plan are valid and timely.  

 

1. A new FSSPFamily Self Sufficiency Plan will be developed if:

 

a. The FSSPFamily Self Sufficiency Plan is not SMART, see WSPM 1002-3, 

b. The Extension Review Staffing indicates a change in circumstances that require changes to assigned activities, 

c. The client cannot or will not do the assigned activities, or, 

d. The FSSPFamily Self Sufficiency Plan activities have been completed. 

 

 

2. The existing FSSPFamily Self Sufficiency Plan is continued if:

 

a. The client is making clear progress towards self-sufficiency, 

b. ATAPAlaska Temporary Assistance Program closure is expected within two to three months based on the current FSSPFamily Self Sufficiency Plan activities, 

c. Changing the current FSSPFamily Self Sufficiency Plan would create new complications for the household, or, 

d. Maintaining the current FSSPFamily Self Sufficiency Plan is a temporary situation to resolve a specific barrier to self-sufficiency. 

 

Document in CMSCase Management System CLNOClient Notess why the changes are being made or why it is in the best interest of the client to continue with the existing FSSPFamily Self Sufficiency Plan.
