1023-6           DISCOVERY ACTIVITIES (INTERVIEWS AND OBSERVATIONS)

  1. The DS meets with the client 3-6 hours weekly over 6-8 weeks (not to exceed 20 hours total) to carry out Discovery interviews and observations. The DS will work with the client to identify FST members to interview as part of Discovery. The DS targets individuals who know the client best.

    1. The CM must continue to contact the DS on a weekly basis (minimum) throughout Discovery to update regarding progress, issues that may impact services or work, and to problem-solve ways to assist clients to complete Discovery appointments and activities.

    2. The CM must summarize Discovery progress each week, any and all activities related to the status of the client’s participation in Discovery visits as CLNOs (see CLNO samples at end of this section).

    3. The DS may call upon the CM for assistance in contacting FST members to arrange interviews. Interviews may take place at any point during the 6-8 week period of Discovery and are typically scheduled for 1 hour or less.

  1. The CM must continue case management and regular contact with the client during the Discovery period.

    1. The CM will carry out all regular case management requirements including phone and personal contact, carrying out FST meetings as needed, and updating the IFSSP.

    2. The CM must invite the DS to all FST meetings during the Discovery period.

    3. The CM must ensure that the DS is given time at each FST meeting to report on Discovery progress and findings based on impact to services or work.

    4. The CM or the FFF may serve as a liaison between the client and the DS if the client is having difficulty engaging in Discovery activities.

    5. The CM will document all client contact and communications associated with the Discovery process as CLNOs.
