1023-5           DISCOVERY INTRODUCTION AND INITIAL VISITS

  1. The CM will conduct an introductory Discovery meeting including the CM, the client and the DS within 2 weeks following Discovery approval. The purpose of the meeting is to acquaint the client with the DS and to provide a full Discovery orientation (see Attachment 1a “Discovery Orientation.”)

    1. The FFF acts as liaison to ensure the introductory Discovery meeting between the CM, the client, and the DS.

    2. The FFF may attend the introductory Discovery meeting, if requested by the CM or the DS.

    3. The CM orients the client using a full Discovery Orientation (see Attachment 1a “Discovery Orientation”) during the introductory Discovery meeting.

    4. At the introductory meeting, the CM provides the DS the completed packet, including the Family Network, Weekly Routines/Calendar, Profile Part I – partially completed, and current release of information forms (ROIs). The CM documents the introductory Discovery meeting as a CLNO (see CLNO sample at end of this section).

  1. During initial Discovery visits the DS meets with the client, and the CM continues necessary case management (the DS is not a case manager) and FST meetings. The CM may be present for initial visits should the DS or client request.

    1. The CM continues case management as needed for service coordination, FST meetings, and assistance to the client in carrying out Discovery activities.

    2. The FFF is available to both the CM and the DS to provide consultation during initial Discovery activities.

    3. The DS may contact the CM for additional information, as needed and with the client’s permission to begin Discovery.

    4. The CM schedules an initial FST meeting after the first week of Discovery visits in order to introduce the DS as a new member of the FST, and to update the FST as to initial Discovery findings. The FFF may be invited to this meeting at the request of the DS or CM as a resource for orienting the FST to Discovery.

The CM will develop a meeting agenda for the DS’ first meeting with the FST that includes the following:

      1. A brief presentation by the DS regarding the process of Discovery

      2. The role of the FST as contributors to Discovery through interviews and giving input to the Profile

    1. The CM must contact the DS as to the status of initial Discovery and keep updated as to the progress of Discovery, specifically any assistance needed to help the client complete appointments or to accompany a client to an appointment.

    2. The CM must document initial Discovery activities in the form of a weekly summary as a CLNO (see CLNO samples at end of this section).
