1023-7           PROFILE DEVELOPMENT

  1. The DS completes a written Profile. The FST contributes to the development of the Profile which is ultimately used as a tool to guide the Employment Planning Meeting (EPM).

    1. The CM works collaboratively with the DS to coordinate, schedule and facilitate a regular FST meeting that incorporates reviewing the Profile as part of the agenda. The CM will document the scheduled meeting as a CLNO.

    2. The CM will distribute a copy of Profile Part II to FST members at least 1 week prior to the scheduled FST meeting. The CM will request that FST members read and review the Profile Part II prior to the scheduled meeting. The CM will copy the Profile Part II as a CLNO and keep a hard copy in the client’s file.

    3. At the FST meeting, the DS will present the Profile Part II and request input from FST members (this will be incorporated into a regular FST meeting, facilitated by the CM).

    4. The DS will develop Profile Part III after the FST meeting, using the input from FST members. Profile Part III will typically be completed within 1 week.

  1. Once the DS completes writing the full Profile, the CM will coordinate, schedule and facilitate the EPM, including the newly designated Job Developer (JD).

    1. The CM will ensure that all individuals invited to participate in the EPM receive a copy of Profile Part III at least 1 week ahead of the meeting. The CM will copy the Profile Part III as a CLNO and keep a hard copy in the client’s file.

    2. At the EPM, the CM will coordinate and schedule a future meeting time (typically within 2 weeks) for Portfolio development, to the include the CM, the JD, the DS and client/job seeker (See Portfolio section).
