1005-3 INCORRECT ITT CONSULTATION REQUESTS
In recognition that no tool can result in 100% correct referrals, we understand that some clients may need to be moved to the alternate track soon after the initial ITT determination. This is primarily for the clients’ benefit. We do not want the referral process to interfere with the clients’ path to self-sufficiency.
During the Employability Assessment (EA) if it appears that a client has been identified or referred to the wrong service delivery track, the receiving agency case manager (the agency to which the client is reporting following the ITT screening will document in client notes (CLNO) the specific indicators that support a incorrect ITT Consultation Request.
The following items must be included in an incorrect ITT Consultation Request but are not limited to:
The EPI and Employability Assessment notes identifying the specific indicators that support the request to transfer the client to the alternate service track.
The Incorrect ITT box checked on the Consultation Request form
The requested service delivery track box must be checked
A list of CMS, CLNO, CANO dates and titles that provide any additional information to support the transfer.
