1023-6         CASE MANAGEMENT AND DISCOVERY SERVICE INITIATION

1023-6 A.     Ongoing Families First Services

Continuity of Work Services regular case management services is an expectation through the provision of Discovery services

1.  Case Management Continues

From the initiation of Discovery through the continuum of services the CM must provide all primary Work Services  case  management to ensure the stability of the FF client/family.

2.  Regular Discovery Services Communication

The DS and CM will contact each other as needed to:

• verify the status of the FF client’s participation in Discovery activities;

• note issues that may impact service needs or work;

• request assistance from the CM to help the FF client complete appointments; and/or

• make service referral recommendations as appropriate.

Note:

The CM must document weekly DS verification documentation as well as noted issues and action plans in CMS.

3.  Identification of Challenges

The DS and the CM must contact one another at any point if issues arise that impact the FF client’s participation in Discovery and work with the CM to problem-solve such issues.

1023-6 B.     Discovery Specialist Contacts CM upon Referral

The DS  will make initial contact with the CM  following the DSC’s receipt of notice of the client’s agreement to participate in Discovery services, formalized in the client’s FSSP or IFSSP.  The purpose of the contact is  to obtain the client’s contact information (ex: phone, message phone, e-mail, postal address, physical address). The CM must familiarize the DS  with the participant’s current services, significant issues and status related to proceeding with Discovery. After the CM has provided the DS basic client contact information, the DS will proceed to attempt to contact the client and initiate an introductory visit within approximately 1 week.

Note:

The CM documents any contact by the DS as a CLNO in CMS.

1023-6 C.     Discovery Specialist Contacts Client upon Referral

The DS will contact the FF client (The DS refers to the FF client as a “participant”) within 2 business days of assignment to inform him/her of the initiation of Discovery services and to schedule the introductory visit. Target date of the first meeting is within 1 week of the DS assignment, at a time and place comfortable to the FF client. The participant is free to invite any natural support or paid service provider to the introductory meeting.

1023-6 D.     Discovery Specialist Confirms Contacts

The DS will inform the CM when successful contact was made with the FF client. If delays occur, the DS must contact the CM and problem-solve ways to encourage DS and FF client contact. The DSC is a liaison to assist problem-solving and ensure the introductory meeting, as well as future visits are carried out.

Note:

CM documents this contact as a CLNO in CMS.

1023-6 E.    Case Manager Packets

In advance of the introductory meeting, the DS  will contact the CM to coordinate a time to pick-up a packet prepared by the CM . The packet will include: a copy of the Family Network, Weekly Routines/ Calendar, and current release of information forms (ROI).
