719-3 ELEMENTS OF THE FAMILY SELF-SUFFICIENCY PLAN
The results of the planning process are documented on the Family Self-Sufficiency Plan form (TA 2) or an employment planning form used by a partner agency. Each FSSP must include:
An employment goal;
A time period for achievement of self-sufficiency;
The steps the family will take toward self-sufficiency;
The services DPA or its agents will provide to assist the family to attain self-sufficiency;
Specific benchmarks or outcomes that indicate progress towards successful completion of the plan;
A statement that the family may be subject to reductions in cash assistance or self-sufficiency services, or other penalties if an adult fails to comply with the plan; and
A statement that describes the necessary conditions and the steps that must be taken to renegotiate the terms of the family’s plan. Plans are updated based on progress in completing steps and achieving goals, and changes in employability.
