719-2          EMPLOYABILITY ASSESSMENT

 

An initial assessment of the employability, strengths, skills, prior work experience, training, and other characteristics affecting the ability to work is required no later than 30 calendar days after determination of Temporary Assistance eligibility for each parent or caretaker who is:

 

 

Information needed to conduct the initial assessment is collected on the Employment Planning Information (EPI) form (TA 5ATAP Work Services Employment Planning Interview). Applicants complete an EPIEmployment Planning Information as a part of the application process. As an alternative, they can supply this information by providing existing similar records from other agencies the family works with. In a two-parent family, both parents must complete separate EPIEmployment Planning Information forms or supply records from other agencies.
