701-9           When A  Family With More Than 57 Months Does Not Want An Extension

In these situation, the case manager attempts to work with the family on a plan for their exit from the program.  The exit plan describes referrals to resources or services available to the family after they leave assistance. A copy of the exit plan and applicable forms is put in the case management file, and a CANO Case Notes notation is made indicating why the family did not request and extension.  

The caseworker is informed of the staffing results and acts to close the case with timely adverse notice.
