MANUAL:  Chronic and Acute Medical Assistance

This is a single subject manual change that revises the review process for ongoing CAMAChronic and Acute Medical Assistance recipients.

 

Beginning January 2011, the X025 notice, CAMAChronic and Acute Medical Assistance Review Application Due, will no longer act as a CAMAChronic and Acute Medical Assistance review application.  Instead, CAMAChronic and Acute Medical Assistance recipients may reapply for assistance by completing a new yellow-colored CAMAChronic and Acute Medical Assistance Review Application (MED 33 ).   The yellow-colored review application form is enclosed with EISEligibility Information System notice X025.  The yellow-colored insert will make it easier for staff to identify when sorting incoming mail.

 

Occasionally, a caseworker may need to manually send a CAMAChronic and Acute Medical Assistance review due notice.  In those situations, EISEligibility Information System notice G801, CAMAChronic and Acute Medical Assistance Review Application Due, is used and the new MED 33 and MED 11 forms are mailed separately.
