This is a single subject manual change that revises the review process for ongoing CAMA recipients.
Beginning January 2011, the X025 notice, CAMA Review Application Due, will no longer act as a CAMA review application. Instead, CAMA recipients may reapply for assistance by completing a new yellow-colored CAMA Review Application (MED 33 ). The yellow-colored review application form is enclosed with EIS notice X025. The yellow-colored insert will make it easier for staff to identify when sorting incoming mail.
Occasionally, a caseworker may need to manually send a CAMA review due notice. In those situations, EIS notice G801, CAMA Review Application Due, is used and the new MED 33 and MED 11 forms are mailed separately.
