126-3 MEDICARE PART D LOW-INCOME SUBSIDY (LISLow-Income Subsidy)
The Social Security Administration (SSASocial Security Administration) has primary responsibility for determining eligibility for the Medicare Part D LISLow-Income Subsidy. The majority of these applications will be process at the Wilkes Barre Data Operations Center in Wilkes Barre, Pennsylvania. Some LISLow-Income Subsidy application may be sent to DPADivision of Public Assistance for processing.
126-3 A. DPADivision of Public Assistance LISLow-Income Subsidy DETERMINATIONS
In the event a LISLow-Income Subsidy applicant insists on DPADivision of Public Assistance making the LISLow-Income Subsidy determination, staff must:
- First, ensure the inquiring applicant is not a Medicaid recipient. If the person is already a Medicaid recipient, there is no need for the applicant to complete the LISLow-Income Subsidy application, as Medicaid recipients are automatically deemed eligible for the LISLow-Income Subsidy.
- If the person is not on Medicaid, provide the applicant with the SSASocial Security Administration -1020B application form.
- After the applicant completes the SSASocial Security Administration -1020B application form:
- Register it in EISEligibility Information System as a General Assistance case to the full service office 030 9 99.
- Upload the application to ILINX for statewide processing (refer to Admin MS 104-1 C about when applications are received by the office).
126-3 B. LISLow-Income Subsidy CASE NUMBERING
- Only one person per case number.
- Register the application as a General Assistance (GAGeneral Relief Assistance) case.
- Couples can apply on the same application; however, each individual must have a separate case number.
