113-9          Q-FORM PROCESS

The Quality Assessment feedback form (Q-Form) is used to report QAQuality Assessment error findings to DPADivision of Public Assistance Leadership, DOSTDivision Operations Support Team, Program Policy, Staff Development and Training, and Program Integrity staff.

The Q-Form is completed by the QAQuality Assessment reviewer to document the circumstances of an error finding during the review process. The narrative will include the specific error element and an explanation of the error determination.

The QAQuality Assessment Supervisor will upload the Q-Form and associated review documents to the QAQuality Assessment SharePoint site in the monthly CAPSCorrective Action Planning Spreadsheets folder.

The Q-Form is also used to notify the responsible eligibility team by email of a household's program disqualification for refusing to cooperate with the QAQuality Assessment review process and will attach the Q-Form to the email.
