The Quality Assessment
feedback form (Q-Form) is used to report QA
error findings to DPA
Leadership, DOST , Program
Policy, Staff Development and Training, and Program Integrity staff.
The Q-Form is completed by the QA reviewer to document the circumstances of an error finding during the review process. The narrative will include the specific error element and an explanation of the error determination.
The QA Supervisor will upload the Q-Form and associated review documents to the QA SharePoint site in the monthly CAPS folder.
The Q-Form is also used to notify the responsible eligibility team by email of a household’s program disqualification for refusing to cooperate with the QA review process and will attach the Q-Form to the email.

This process requires forwarding of the e-mail and attachments.
