3001-1 APPLICATION FORM AVAILABILITY
An applicant must apply for heating assistance benefits using either the DPA HAP application form (HAP1) or by completing the telephonic application submission process. The form must be signed by the head-of-household, or an individual authorized by the household to act on its behalf. The Heating Assistance Program pre-mails applications directly to all households that received heating assistance in the previous year and to those who have contacted the program to request one. Applications are also available at or from: all DPA offices, Fee Agents, home heating suppliers, senior centers, Special Supplemental Food Program for WIC clinics, Head Start Centers, concerned agencies, the internet at www.heatinghelp.alaska.gov and by mail at:

Application forms are pre-mailed to the elderly and disabled in late-September, to families with young children in early-October, and to all other participants from the previous season in mid-October.
