1024-4           EPM FOLLOW-UP

• The Case Manager coordinates a meeting approximately 2 weeks out with the job seeker, the DS Discovery Services and the JD Job Developer in order to carry out development of the Portfolio.

• The Case Manager is responsible to transfer EPM Employment Planning Meeting meeting notes (from wall paper) to formal typed notes.

• The Case Manager confirms the notes with the job seeker 1-2 days after the EPM, then sends the final EPM meeting notes and a copy of the Employment Plan to the attendees.

• The Case Manager transfers the EPM meeting notes and Employment Plan as a CLNO Client Notes.

• The JD Job Developer will contact the identified “door openers” from the Employment Plan as he/she proceeds through the phase of negotiated job development.

• The CM continues case management as needed for service coordination, FST Family Support Team meetings, and assistance to the client in carrying out Discovery activities.
