  1. The case manager arranges FST meetings, and encourages attendance of members.

    1. The case manager must work with all FST members to ensure a meeting date, time and place that results in the highest attendance by accommodating member availability.

    2. The case manager must secure appropriate space and supplies for each FST meeting.

  1. The case manager must develop a meeting agenda that is responsive to the client and family’s needs, as well as agency priorities and input. The agenda is included in a formal meeting invitation to FST members.

    1. The case manager must coordinate meetings based on the client and family’s current and changing needs and circumstances.

    2. The case manager will invite FST members based on their contribution to a specified meeting agenda. The case manager may assist the client to invite their natural supports.

    3. The case manager must document all meeting agendas as a CLNO in CMS.
