1017-3 CONDUCTING THE HOME VISIT
Case manager’s responsibilities are to:
Arrive on time and conduct the home visit within the established time allotment.
Keep the home visit on task by keeping to the topics previously established.
Introduce or re-introduce any other participants involved in the meeting and clearly define their role.
Give the client an opportunity to express their concerns and ask questions.
Give very clear answers to questions and if you don’t know the answer, be sure to find the correct answer and follow-up with the client at a later date.
Do what you say you are going to do on behalf of the client.
Document the home visit and what transpired.
