1017-1 HOME VISIT GOAL
Each home visit should have a defined goal with specific tasks to complete and specific topics to discuss.
Set the home visit up with a goal of working toward self-sufficiency.
Involve the client in the planning of the home visit.
This can be a part of the client’s FSSP or re-engagement.
The home visit should be planned with the client, not for the client.
Work on building trust with the client or family in preparation for the home visit as well as during the home visit.
Keep the tone of the home visit upbeat and positive.
If the environment seems tense do your best to “break the ice.”
Make positive statements.
Example:
Tony arrived for a scheduled home visit with his client. Upon
arrival he noticed the home was in disarray, cluttered and the energy
of the home was somewhat chaotic. Tony
quickly realized that his client was embarrassed and uptight at the condition
of the home. He noticed
family pictures hanging on the wall in the living room and made the following
statement: “Those are beautiful pictures, are those all of your children?” After a brief discussion
of the photos, the client suggested they sit on the sofa in front of the
coffee table to have their home visit. Tony
noticed a beautiful quilt covering the back of the sofa and made the following
statement: “This is a beautiful quilt, did you make it yourself?” Again, after another
brief conversation about the quilt the client seemed more at ease and was better able to discuss
her family dynamics, interest, hardships, successes, etc.
Show an interest in the family and do not give the impression you are evaluating the home. It is important to try and have the client feel at ease about the home visit.
Engage in more listening and less talk.
Look beneath the surface during the home visit. Try to identify any underlying issues that might contribute to a client’s inability to complete assigned activities.
Leave the home visit with a plan; reiterate with the client the decisions that were made and any follow-up items.
Schedule your next appointment.
Assure documentation is completed.
