1004-3           DUE DILIGENCE OF THE WORK SERVICES CASE MANAGER

Due diligence of each case manager is measured by the documentation and progress of each client toward their self-sufficiency goals.

        • Follow all DPA Policy and Procedures as outlined in the ATAP Manual and Work Services Procedure Manual;

        • Conduct interviews, assessments and evaluations with each client;

        • Seek collaboration with partners to support each client’s unique strengths and needs;

        • Create ways to coordinate activities to increase client success;

        • Complete referrals appropriately and through the DPA designated process;

        • Measure the client’s progress;

        • Assure the client’s privacy is maintained; a signed release of information may be required.

        • Document all client activities.
