1004-3 DUE DILIGENCE OF THE WORK SERVICES CASE MANAGER
Due diligence of each case manager is measured by the documentation and progress of each client toward their self-sufficiency goals.
Follow all DPA Policy and Procedures as outlined in the ATAP Manual and Work Services Procedure Manual;
Conduct interviews, assessments and evaluations with each client;
Seek collaboration with partners to support each client’s unique strengths and needs;
Create ways to coordinate activities to increase client success;
Complete referrals appropriately and through the DPA designated process;
Measure the client’s progress;
Assure the client’s privacy is maintained; a signed release of information may be required.
Document all client activities.
