127-6          CHILD NEEDS TRANSPORTATION TO AN OUT-OF-STATE TREATMENT FACILITY

 

A request may be received to send a child to an out-of-state treatment facility.  Out-of-state placements are not approved unless the child is certain to remain there for 30 days or longer.  The following steps must be made:

 

  1. The treatment facility requests prior approval from First Mental Health.

 

  1. First Mental Health reviews request, and if approved, certifies with a Prior Authorization (PA) number, and notifies DMA , who will in turn notify the appropriate caseworker.

 

  1. Consider the child living separately from his or her parent(s) and authorize a child only DKC case.  A DE 25 coupon is not necessary.

 

Note:

The caseworker should still request the Med 18 and retain in the casefile.
