109-7           CANO Case Notes FOR SPECIAL CIRCUMSTANCES

For tracking purposes, a separate case note is necessary when documenting special circumstances. Staff must include a specific and descriptive title for such case notes. Some examples are:

1. • Medicaid Citizenship and Identity

   • Transitional Medicaid (12 Month Continuous Eligibility Period)

   • Fair Hearing Requests

   • Prehearing Conferences

   • Fraud Referrals

   • Case Correction for QARC Quality Assessment Review Committee Errors

The following situations may sometimes be included in the intake CANO Case Notes and not as a separate CANO Case Notes entry. However, if the issue occurs in the middle of the certification period, a separate CANO Case Notes entry should be created:

1. • ATAP Incapacity Determination

   • Drug or Other Felony Convictions

   • Unusual Alien situations
