116-3       VERIFICATION

Applications received from an NFAP Native Family Assistance Program should include copies of documentation and verification the NFAP Native Family Assistance Program worker obtained from the applicant.  If additional information or verification is needed to process the application, the DPA Division of Public Assistance eligibility worker will contact the applicant to get this information.

DPA Division of Public Assistance may accept and use information provided on NFAP Native Family Assistance Program forms such as employment and income information, landlord statements, shelter deductions, household composition, and child support.
