After an NFAP employee has read the DPA Security Manual and taken the HIPAA Training, they can complete and submit their requests for access to the State of Alaska Data and Information Systems.  Each system is owned, maintained and operated by separate State of Alaska Departments, Divisions and IT personnel.

 

For staff that are performing jobs that involve eligibility for TANF benefits, there are five forms that are needed:

 

1. The Non-State Employee Access Request Form (two pages)

This form is required for an NFAP employee to be assigned a State of Alaska and Dept. of Health and Social Services account, user ID and password.

 

2. The ETS Mainframe Logon ID Request Form (four pages)

This form is required for an NFAP employee to be assigned a Mainframe ID, which is used to enter the State of Alaska's Mainframe and then the Dept. of Health and Social Services' EIS or the Dept. of Revenue's Child Support System (NSTAR).

Note:

For access to the Child Support Services Division's NSTAR database, you need to mark the box next to: "XJUPDMSS PROD DB193"


3. The DPA Security Agreement (one page)

This form is required for an NFAP employee to be granted access to any DPA case information in EIS, ARIES or any other associated program.

 

4. The Dept. of Labor (DOL)/LAN Work Order and Customer Acknowledgement Page (three pages)

This form is required for an NFAP employee to access the Dept. of Labor's SAM System.

 

5.  The CSSD External Users Confidentiality Acknowledgement Form (one page)

This form is required to access the Department of Revenue's NSTAR system.
