812-3        HOW A CARETAKER COOPERATES WITH CHILD SUPPORT SERVICES DIVISION

 

A parent or guardian cooperates with CSSDChild Support Services Division by:

 

1.  Completing and signing the Child Support Information form for each noncustodial parent not living in the home of a dependent child;

 

2.  Helping to identify and locate the noncustodial parent of each child included in the household;

 

3.  Appearing at a CSSDChild Support Services Division office when requested to provide information or evidence relevant to the case;

 

4.  Appearing as a witness in a court or other proceeding;

 

5.  Providing information or attesting to lack of information under penalty of perjury or unsworn falsification; and

 

6.  Taking any other reasonable steps to assist in establishing paternity and securing medical support.

 

If the parent or guardian relative fails to cooperate with CSSDChild Support Services Division in any of these ways and does not establish good cause for not cooperating, a penalty to the parent or guardian will be imposed.
