1021-7 FAMILY SUPPORT TEAM DOCUMENTATION - DUE DILIGENCE
Documentation of FST activities, communication, participation, identification of service coordination needs and client and family progress is required, as well as to identify service successes and gaps with the entire DHSS service delivery system. Due diligence is demonstrated through proper documentation by the case manager.
The case manager must document all communication and attempts to contact via a variety of means. Due diligent effort to communicate and engage a FST member must show use of 3-5 different methods to contact, including but not limited to: phone exchanges, voicemail/recorded messages, postal letters and notices, e-mail, text messages and hand-delivered notes and announcements. All efforts to communicate must be documented as a CLNO in CMS . The type of communication must be identified, including the number of attempts to contact.
The case manager must document as a CLNO all FST meeting invitations.
The case manager must document as a CLNO all communications with FST members that address the client’s status or client/family circumstances.
The case manager must ensure that a FST member takes meeting notes that include the following, at a minimum (see Families First Work Services training materials):
participants (by name, agency or role)
successes or progress
rationale for continuing existing services
new service needs or identified barriers to services
strategies to meet service needs or to overcome barriers
client expectations
a concise plan of action including measurable strategies to meet the client’s health, safety, well-being and self-sufficiency goals (WSPM 1008)
date of next FST meeting
All FST meeting notes are copied and pasted as a CLNO , and reference made to the date meeting notes were sent out to FST members.
