1017-5 HOME VISIT DOCUMENTATION
Comprehensive documentation is imperative to this process. Client related notes can be used in a Fair Hearing process. All forms and documentation must be adequate, relative and comprehensive. The Case manager’s responsibility is to:
Document in CLNO ’s all activities involved in arranging and facilitating the meeting, topics discussed, decisions made, follow-up activities, etc.
Maintain all relevant hard copy documentation in a chronological order in the client file.
Ensure the newly developed or revised FSSP /Informed FSSP (WSPM Section 1008) is comprehensive, has appropriate goals, is agreed upon by the client and any other relevant partner agencies, and is signed by all parties involved.
Resolve any follow-up activities and document accordingly.
Adequately summarize key points gained by the home visit and appropriately adjust client activities to support self-sufficiency.
