1004-1           DUE DILIGENCE GOAL

It is expected that the level and type of service provided to families is based on an assessment of their capacity to participate in activities and the type and frequency of supports they need to become self-sufficient.

The following are required activities:

  1. Use of all prescribed assessments, screening tools and evaluations (WSPM Section 1004).

    1. Initial Tracking Tool (Attachment)

    1. Alaska Behavioral Screening Tool (AST)

    2. Employment Planning Information (EPI)

    1. Release of Information Forms, as appropriate.

  2. Complete an Employability Assessment within 30 days of client referral for FF or WF services (WSPM Section 1007);

  3. Complete of an FSSP for the WF track or an IFSSP for the FF track within 30 days of client referral for Work Services (WSPM Section 1008);

  4. Quarterly and Annual Review Staffings are required every 3 months (WSPM Section 1016);

  5. Collaboration to develop the self-sufficiency goals, steps and plan for each client is required. FF clients require that case managers develop, and sustain a Family Support Team. (WSPM Section 1021);

  6. Identification and assignment of appropriate work and other activities that support client success.

  7. Home or community visits are required when the client needs a broader range of services or when meeting in the Work Services office is challenging (WSPM Section 1017).

  8. When clients are transferred from one service track to the other all required documents and steps must be completed. Note: This should not be used as a check-list but as a guide to determine if due diligence has been met. There may be other steps to guide the Case Manager to exhibit “due diligence” in specific cases with unique circumstances.
