717-1 INFORMING APPLICANTS AND RECIPIENTS ABOUT CHILD SUPPORT SERVICES AND REQUIREMENTS
When an initial application is received and when a recipient requests that a child be added to the case, the caseworker must discuss child support assignment, cooperation and enforcement requirements with the caretaker relatives who are required to cooperate and provide them with:
A Child Support Information form (CSSD 1603a) to complete for each noncustodial parent not living in the home of the dependent child;
Written notice of the caretaker's right to claim good cause for not cooperating with CSSD (provided on the DPA application and the Child Support Information form);
A Child Support Cooperation Statement form (GEN 80 or EIS notice W060) for caretakers to complete if they choose not to provide the information requested on the Child Support Information form and want to claim good cause; and
An opportunity to request that CSSD keep the family’s address confidential because of a threat to their well-being. The Release of Information form (CSSD 0500b) is used for this purpose. In addition, CSSD will provide the caretaker with an Affidavit for Address Confidentiality when the child support case is opened. These forms are used to keep identifying information from being released to a person (such as a parent) who would otherwise be entitled to have information about the child support case.
