126-2 SENIOR BENEFITS OFFICE
The Senior Benefits office is a statewide office located within the Mat-Su DPA District Office in Wasilla. The Senior Benefits Office is responsible for the following programs:
Medicare Part D Low-income subsidy benefits
Senior Benefits Program
The Senior Benefits office is the Full Service Office number 030.
The Senior Benefits Program has a different application form than other Public Assistance programs. Most Senior Benefits applications will be mailed directly to the Senior Benefits Office, however, a Senior Benefits application will be accepted by any DPA office. If received in another DPA office, staff must:
Register the application as a Request for Service (RG) to 030 9 99.
Forward the registered application with a CAP1 and CAP2 attached to the application to the SeniorCare office located within the Mat-Su district office.
126-2 B. SENIOR BENEFITS REGISTRATION AND CASE NUMBERING
Register Senior Benefits cases using a General Assistance case number. Senior Benefits cases must have a new and separate case number that cannot be used for any other programs within DPA . If both members of a couple are applying for Senior Benefits, each person must have their own case number.
Example:
Mrs. Smith received General Assistance for utility shut-off in November 1998. Mrs. Smith and her husband each submitted an application for Senior Benefits. When registering the application two new case numbers must be established for Mr. and Mrs. Smith. The old General Assistance case number cannot be recycled as a Senior Benefits case number.
