126-3 MEDICARE PART D LOW-INCOME SUBSIDY ( LIS )
The Social Security Administration ( SSA ) has primary responsibility for determining eligibility for the Medicare Part D LIS . The majority of these applications will be processed at the Wilkes Barre Data Operations Center in Wilkes Barre, Pennsylvania. Some LIS application may be sent to DPA for processing.
126-3 A. DPA LIS DETERMINATIONS
In the event a LIS applicant insists on DPA making the LIS determination staff must:
First, ensure the inquiring applicant is not a Medicaid recipient. If the person is already a Medicaid recipient there is no need for the applicant to complete the LIS application, as Medicaid recipients are automatically deemed eligible for the LIS .
If the person is not on Medicaid provide the applicant with the SSA -1020B application form.
After the applicant completes the SSA -1020B application form:
Register it in EIS as a General Assistance case to the full service office 030 9 99.
Forward the completed application with a CAP1 and CAP2 attached, to the Senior Benefits Office located within the Mat-Su District Office.
126-3 B. LIS CASE NUMBERING
Only one person per case number.
Register the application as a General Assistance ( GA ) case.
Couples can apply on the same application; however, each individual must have a separate case number.
