105-8 A.      DEFINITION OF A RELEASE OF INFORMATION

 

A release of information is a statement signed by the client authorizing a contact person to give the division information about the client's situation.

 

A generic release of information allows the caseworker to contact a variety of agencies and institutions such as financial institutions, Native corporations, landlords, employers, and school authorities.

 

A specific release of information form is one that lists the name of one specific source, such as the National Bank of Alaska.

 

105-8 B.      WHEN A RELEASE OF INFORMATION FORM IS REQUIRED

 

A release of information form is not required to attempt a collateral or other third party contact. A release of information form may be required, however, if the contact person refuses to provide the information without a release.

 

The caseworker may request that the client complete a release of information form during a face-to-face interview, during a home visit, on the telephone, or in writing. Often, caseworkers request releases from clients so that one will be on file for future use. This is an acceptable practice.

 

105-8 C.      WHAT TO DO IF A CLIENT DOES NOT COMPLETE A RELEASE OF INFORMATION FORM

 

If a client does not provide a signed release of information form, the caseworker should explain what information is needed or which eligibility criteria must be verified.

 

Benefits may not be denied or discontinued because a client did not complete a release of information form. All clients are required to furnish information and verification necessary to establish initial and continuing program eligibility. Benefits may be denied because a client did not provide requested information or adequate verification that he or she met certain eligibility criteria.
