105-3          DOCUMENTARY EVIDENCE

 

When verification is required, caseworkers use documentary evidence as the primary source of verification. Documentary evidence consists of written confirmation of a household's circumstances. Examples of documentary evidence include:

 

 

Although documentary evidence is the primary source of verification, acceptable verification will not be limited to a single document or source. Whenever documentary evidence cannot be obtained, the caseworker may use alternate sources of verification such as collateral contacts and home visits. In all cases, the method of verification is recorded in the case record.

 

Note:

Eligibility staff may request to order out-of-state birth certificates if the client is unable to provide a copy. To request out-of-state birth certificates, eligibility staff should contact their region's Administrative Assistant. The Administrative Assistant will place the order and pay for the cost of the birth certificate.
