The NFAP Point of Contact (POC) must print and use this checklist when a staff member no longer needs system access. Failure to remove access for someone who is no longer employed or has changed jobs and no longer needs access to do their job is a security violation. The checklist and all of the forms/ documentation shown on the checklist must be complete prior to submission to the DPA sponsor at hss.dpa.policy@alaska.gov. The POC must keep a copy of these forms and certificates for audit purposes. If there is more than one person that needs their access removed at the same time, one email can be sent with the forms for each attached.

◻  Non-DHSS Employee Access Request Form (VPN)

◻  ETS Mainframe logon ID Request Form

 

If the individual is no longer employed, please put “Not available for signature” anywhere the employee would typically sign the form.
